Application Forms

You can complete and submit a credentials evaluation application to AERC by clicking on one of the formats below:

• Adobe PDF format* (print application, fill out by hand, mail to AERC)
• MS Word format (fill out via MS Word, print completed application, mail to AERC)
• HTML format (fill out online, print completed application, mail to AERC)

Please be sure to check and complete all questions. The evaluation cannot be completed until all required documents and fees have been received by AERC.

Additional Forms

Applicants who have an evaluation on file may request an additional official evaluation report by completing the “Request for Additional Official Copies Form" and mail it to AERC with the appropriate fee.

For additional evaluation reports, please fill out one of the following forms:
